Background:Effort to relax the blood vessels in the human penis is related to penile erection. Erectile dysfunction (ED) occurs when the penis is not able to attain adequate erection for satisfactory sexual intercourse. Erectile dysfunction is not a general condition, but it is one of the most common sexual dysfunction among men in menopause. In such cases, the cause is unknown. ED affects around 1 in 10 men and is a common disorder that occurs in 6% of men, and approximately 3.6 million men worldwide. Erectile dysfunction is common in menopausal women and is associated with a range of physical and psychological factors. This review describes the prevalence and causes of ED among menopausal women and the current knowledge regarding its prevalence in menopausal women.
Objectives:To evaluate the prevalence and associated causes of erectile dysfunction among a sample of menopausal women and their current knowledge regarding the prevalence and causes of their current ED.
Setting:Healthy Aging System (HAS), a population-based, prospective, cross-sectional study at The University of Texas Southwestern Medical Center, Dallas, Texas, USA. The study was carried out in HAS (Harmonized Women) and in the US and UK (United Kingdom).
Participants and methods:This prospective study was approved by the Institutional Review Board of HAS (approval number: IRB-15-061).
Study population:A total of 943 menopausal women aged 50 to 69 years, with a mean age of 51 years (SD=20.0), who participated in the study were recruited from women who did not currently have erectile dysfunction (ED), or who did not have an active lifestyle or medical history of ED. The women were invited to participate in a medical questionnaire to gain a detailed understanding of their ED and their current symptoms. All participants were randomly assigned to take either a placebo pill (10 mg Vardenafil) or a combination of two placebo pills (100 mg vardenafil) for one year. After the first year, all participants had to be treated for one month with Vardenafil, while the other pill was taken for another year. After the study was over, the participants continued to take either Vardenafil (10 mg Vardenafil) or placebo pills. The results showed that the average duration of ED treatment with the two placebo pills was 4.6 months (range, 1.3 to 6.9 months). The average duration of ED treatment with Vardenafil was 3.9 months (range, 1.2 to 6.9 months).
Main findings:The prevalence of ED among menopausal women is approximately 1.1% (n=1,074) and in HAS (n=943), the prevalence was 1.7% (n=903). The prevalence of ED among the general population was significantly higher than that in menopausal women (1.1%) (95% CI, 1.3% to 1.5%). The most common underlying cause of ED is smoking (1.3%).
Conclusions:The current knowledge regarding the prevalence and causes of ED among menopausal women and the current knowledge regarding the current prevalence and causes of their current ED is limited. In the USA, the prevalence of ED was found to be 2.5% and in the UK, it was 1.6%. The current knowledge regarding the prevalence and causes of ED is limited.
